• Application / Fee / Roster (Unofficial needed for Pointstreak) - deadline extended to Tuesday, Sept 7th
  • Confirmation on Thursday, September 9th.
• A mandatory credential check will be held for all teams/divisions prior to your first scheduled game.
  
• Check includes:
  (we don't need a copy - we only need to check that you have the documentation)
  
  • Official USA Hockey player roster for the 2010-2011 season signed by associate registrar;
  • Signed copy of each player's USA Hockey Code of Conduct form and
  • Signed copy of each player's USA Hockey Consent to treat 
  • Note: Coaches must have a current card or certification or will not be allowed on the bench.
